The policy below applies to Perry Chiropractic Health Center (hereinafter referred to as “Perry Chiropractic” internet website, www.perrychirowellness.com).
Perry Chiropractic is committed to protecting your online privacy. In general, you can visit Perry Chiropractic’s internet webpages without revealing any personal information.
Any personally identifiable data that you enter on this site is not sold to third parties.
Personally Identifiable Information & Protected Health Information
We implement the following as a matter of sound business practice; to protect the interests of our customers; and to fulfill our legal obligations under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”), its implementing regulations at 45 CFR Parts 160 and 164 (65 Fed. Reg 82462 (Dec. 28, 2000)) (“Privacy Rules”), as amended, and state law that provides greater protection or rights to patients than the Privacy Rules.
Protected Health Information (PHI) and Personally Identifiable Information (PII) refer to information which can be used to distinguish or trace an individual’s identity, such as their name, address, date and place of birth, mother’s maiden name, social security number, biometric records, credit card number, driver’s license, medical records, etc.

Perry Chiropractic only uses PHI/PII to deliver the requested services and for internal purposes, such as monitoring, quality control, and process improvement. Using Third-Party Tools for Outreach through Digital Advertising
Perry Chiropractic uses third-party web services to conduct outreach and education through the use of digital advertising. These services may receive your PHI/PII as part of outreach, education, or other communications authorized by Perry Chiropractic. These third-party services may collect information through the use of web beacons that are located on our pages. A web beacon is a see-through graphic image (usually 1 pixel x 1 pixel) that is placed on a web page and, in combination with a cookie, allows us to collect information regarding the use of the web page that contains the web beacon.
